PSYC 389 Experiential Learning Contract

 
This form is used to apply for enrollment in PSYC 389 Experiential Learning, a course through which undergraduate psychology majors can earn academic credit for a professionally supervised internship experience. Students are responsible for independently arranging the terms of their internship, and an appropriate site professional must agree to supervise your independent study BEFORE you can apply for enrollment in PSYC 389 Experiential Learning. 
Students are NOT permitted to intern within a department or unit of an organization that employs or has on its volunteer roster a family member, friend, or relative. Family Members, friends, and relatives may also NOT serve as Internship Site Supervisors. Students are also NOT permitted to serve as an intern with an agency/organization through which they have previously served as a client.
Before You Begin
 
This application will ask you specific questions about your internship. These questions include details about you responsibilities, the learning objectives of the internship, and the internship site's expectation of you. All students are expected to have met with their site supervisor and understand his/her expectations prior to completing this application.
The Process
 
When you submit the application, an email will be sent directly to your site supervisor. He/she will review the content of your application at that time. Your supervisor will approve the application by replying to that email. This process may take several days, so please plan to submit your application well before the last day of schedule adjustment. All applications must be approved by the site supervisor by 10:00am on the last day of schedule adjustment for the semester during which you will be participating in the internship. We will not be able to process applications after this deadline.
